Castell care coordinators were manually reviewing approximately 100 patient charts per day in iCentra Cerner EHR to identify VBC payers and validate care gaps, spending ~3 minutes per chart on point-and-click data entry. The process required toggling between filters, locating keywords and dates, reviewing scanned reports and faxes, and making outbound calls — consuming an average of four hours of each coordinator's day in administrative work.

Castell deployed Notable's Intelligent Population Health solution, using automated digital assistants (bots) to perform chart review work autonomously. The bots identify interval follow-ups, reconcile missing data, and surface actionable care gap information directly to care coordinators — eliminating manual EHR navigation and enabling coordinators to focus on patient engagement rather than data entry.

Chart review time dropped 67%, from four hours to one hour per day on average. The automation saves 1.68 minutes per chart and enables coordinators to review charts for 3,362 additional patients daily. The equivalent workload of 34 full-time care coordinators — representing more than 190 hours of work — is now handled automatically, and the solution went live in just four weeks.
