The Challenge
Maine is one of the most rural states with the oldest population by median age. Northern Light Health needed to identify and prioritize COVID-19 treatment delivery to high-risk patients across a geographically dispersed region covering roughly two-thirds of Maine, encompassing 10 hospitals and 100+ facilities.
The Solution
The health system developed a risk stratification tool within Oracle Health EHR that analyzed data from COVID-positive patients and assigned a numeric risk score. An Incident Command Center was formed to notify high-risk patients and deliver monoclonal antibody treatment close to home. Oracle Health also helped develop a faster vaccine throughput workflow to scale delivery.
Results
